Ultra Care Family Wellness is a Brooklyn-based home-care agency helping seniors, adults with disabilities and their families understand options and organize what comes next across NYC and Nassau County.

Ultra Care started because families across Brooklyn kept discovering that the hardest part of home care wasn't the caregiving — it was the system around it.
Assessments, authorizations, plan rules, renewal deadlines and document requests arrive in language designed for administrators, not for a daughter working two jobs.
We built an agency around that gap. Today our clinical, benefits, OPWDD and recruitment teams handle the coordination from our Brooklyn office so families across NYC and Nassau County can spend their attention on the person who needs it.

We can collect discharge information, expected home-care needs, preferred start date, equipment concerns, and family availability to support a safer transition home.
We help families identify useful documents such as insurance cards, authorizations, assessments, discharge papers, doctor information, and caregiver certificates.
We help families understand the intake status, missing information, follow-up requests, scheduling discussions, and next actions without making promises about approvals.
Seniors aging in place, adults recovering from hospitalization or surgery, people with disabilities, children and adults eligible for OPWDD services, and the family members coordinating it all.
