No guarantee of eligibility, hours or employment
Submitting any form on this site — including intake, appointment-request, OPWDD, Medicaid-assistance or employment applications — is a request for review only. It does not guarantee Medicaid or program eligibility, approved service hours, caregiver approval, placement, or employment.
Eligibility, authorized hours and program approval are determined solely by the responsible health plan, government program or agency, not by Ultra Care Family Wellness.
