Information we collect
When you use our intake, employment, OPWDD, Medicaid-assistance, appointment-request or contact forms, we may collect information you choose to provide — such as your name, date of birth, phone number, email, address, borough, insurance or MLTC details, Medicaid status, care needs, employment history and related documents.
We also collect limited technical information automatically, such as browser type and general usage patterns, to keep the site working well.
How we use it
Information submitted through our forms is used to understand your situation, prepare a possible next step, and allow a care advisor, benefits specialist, OPWDD lead or recruiter to follow up.
We do not use the information you submit for automated eligibility decisions, and submitting a form never guarantees eligibility, service hours, caregiver approval or employment.
Demo forms and data transmission
Important: the forms on this website are currently built as a front-end demonstration. Unless and until a compliant, secured backend is connected, form submissions are not transmitted to or stored on a remote server — they exist only in your browser session and are discarded when you close or reload the page.

No sale of information
We do not sell, rent or trade personal information to third parties for marketing purposes.
Information may be shared only with your consent and only as necessary to coordinate care — for example, with a health plan, case manager, physician or hospital discharge team you have authorized us to contact.
Retention
Once a compliant backend is connected, information related to an active intake, application or care relationship will be retained only as long as reasonably necessary for coordination, recordkeeping and applicable regulatory requirements, then securely deleted or archived per policy.
Security
We intend to apply industry-standard administrative, technical and physical safeguards to any information collected once live data handling begins. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
Children's information
Our OPWDD-related forms may involve information about a minor child submitted by a parent or guardian on the child's behalf. We do not knowingly collect information directly from children, and all child-related submissions should be made by a parent, guardian or authorized representative.
