Getting started
In most cases care can begin within 24 to 48 hours of your first call, depending on authorization and staffing. For urgent or post-hospital situations, same-day starts are often possible — call us directly so we can move immediately.
Our office is at 147 Front Street, Unit 9 & 13, Brooklyn, NY 11201. Walk-ins are welcome by appointment — call ahead so the right specialist is available when you arrive.

No. We assist with intake, documentation, coordination and understanding available pathways. Eligibility, authorized hours and program approval are decided by the responsible plan, program or agency.
You can request a reassessment. We help document changed needs, hospitalizations, safety concerns and gaps in support, and explain how to request review through your plan. Additional hours are never guaranteed.
Yes. Our benefits specialists help assemble identity, residency, financial and medical-necessity documentation for a Medicaid or Medicare Savings Program application. We coordinate the paperwork; the responsible agency makes the eligibility decision.
OPWDD & family caregiving
Yes. We guide families through the OPWDD Front Door process for children and adults, including eligibility documentation, the intake meeting and care-management enrollment. We do not determine OPWDD eligibility ourselves.
Sometimes. Certain programs allow eligible consumers to select approved relatives as personal assistants, and separately you can train as an HHA or PCA. Relationship restrictions, training, screening and program approval all apply.
Care team & aides
Yes. Every aide is background-checked where applicable, license-verified, health-cleared and trained in our person-centered methodology. Memory-support aides complete additional specialty training.
Yes. We match aides on language, schedule, location and care needs where staffing allows, and families are introduced before the first shift whenever possible.
Care plans are flexible. Schedules and services can be adjusted through your plan and our coordination team, and there are no long-term private contracts.
