Grace guides families through Front Door intake, eligibility documentation and self-directed budgets, coordinating closely with care managers across the five boroughs and Long Island.

Yelena helps families assemble Medicaid and Medicare Savings Program applications, tracks renewal deadlines and prepares documentation packages for review.

Joseph runs the aide portal — HHA and PCA pathways, CNA transitions, onboarding documents and family-caregiver inquiries — with a bias toward getting people working quickly and correctly.
